Prior Authorization Letter

The Prior Authorization letter is now sent by the Division. Molina discontinued sending letters to providers for the prior authorization status. The emails are sent out to providers for participants with services that have the prior authorizations processed by Molina and received at the Division.

It is possible that there may be prior authorizations processed for more than one service and the status letter will contain the prior authorization status details of both these services.

Each document includes a unique Doc ID, which is on the bottom left of the footer, and users are advised to refer the Doc ID for any communication with the Division. The Reviewer ID and Review Date currently do not display any information but this is subject to change in the future.

For each individual service, the prior authorization status report contains the following details, shown in the highlighted sections of Supports Program and CCP status letters:
